Dr. Jay Connelley, MD, of Monticello is opening a new office for his medical practice, according to a news release.
“As of Jan. 1, 2019, Dr. Jay Connelley will open the doors of Connelley Family Medical PLLC at 777 Jordan Drive (adjacent to Reinhart Family Healthcare). He will no longer be seeing patients at the Monticello Medical Clinic,” according to the release.
His current patients will need to complete a medical record release form requesting original records to be transferred. The form can be obtained from Connelley Family Medical after Jan. 1 or at the Monticello Medical Clinic, according to the news release. Patients charts will remain at MMC for all patients without completed forms.
Connelley considers it a privilege to practice medicine for the past 20 years in Monticello, he said in the release. He and his staff look forward to continuing to provide medical care for his patients and families.
