The Arkansas Department of Health regulates the sale of food at establishments that include restaurants, bars, day cares, schools, grocery stores, convenience stores, bed and breakfast facilities, hotels and motels, mobile units and concession stands. Inspections are made unannounced by environmental health specialists.

TAYLOR ELEMENTARY SCHOOL, 805 West St., White Hall. Date of inspection Aug. 27. Observed boxes of dry storage foods being stored directly on the floor. Boxes of food should be stored at least 6 inches off the floor. Crates were placed under the boxes during inspection.

COVENANT RECOVERY INC., 3201 W. Pullen St. Date of inspection Aug. 25. Observed cereal stored directly on the floor. Food must be stored 6 inches above floor to prevent contamination. Observation: Facility lacks test strips. Corrective Action: Test strips must be available and used to test the strength of chemical sanitizing solutions.
CHEF KING INC., 2318 W. 28th Ave. Date of inspection into complaint Sept. 2. No violations reported.
ALTHEIMER FOOD MART LLC, 13512 N Highway, Altheimer. Date of inspection Sept. 1. Observed inside of refrigerator with accumulation of food residue. Observed pests throughout the establishment. The premises shall be free of insects, rodents, and other pests and measures must be taken to to eliminate their presence. Observation: Walls and ceilings are soiled in kitchen area. Nonfood contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Observation: Floors in kitchen area are stained and need to be cleaned. The establishment shall be cleaned to sight.
COUNTRY KITCHEN, 4322 Dollarway Road. Date of inspection Aug.
- Chlorine solution is not dispensing properly to sanitize the dishes in the automatic dishwasher. A chlorine solution shall measure 50-100 mg/L at a temperature of 75°F-100°F. Dishwasher was repaired so that the sanitizer properly dispenses. Ham (43 degrees F) and potato salad (48 degrees F) in back reach in refrigerator and ranch dressing (46 degrees F) in front sliding door refrigerator are out of safe temperature range. Time/Temperature Control for Safety (TCS) Foods shall be kept cold at a temperature of 41°F or below, as measured by an internal probe thermometer, to limit bacterial growth. Observed food prepared on site in refrigerators held for more than 24 hours and not date marked. Food prepared on site and held in refrigerator for more than 24 hours should be date marked and discarded after seven days. Observed boxes of food being stored directly on the floor in the back walk-in freezer and walk-in refrigerator. Boxes of food should be stored at least 6 inches off the ground. Single-service containers observed being reused throughout establishment. Single-service containers should not be reused. Shelves in kitchen and dry storage area are unclean and need to be cleaned. Observed trash cans containing food residue being stored uncovered while not in continuous use. Trash cans containing food residue should be covered while not in continuous use. Floors throughout facility, especially under equipment and shelving, are unclean and need to be cleaned. Clean the physical facilities as often as necessary to keep them clean.
LAVENDER’S BARN, 4501 U.S. 65 South. Date of inspection Aug.
- Sliced tomatoes (75 degrees F) in prep area and beans (50 degrees F) and buttermilk (49 degrees F) in the reach in refrigerator in the kitchen are out of safe temperature range. Time/Temperature Control for Safety (TCS) Foods shall be kept cold at a temperature of 41°F or below, as measured by an internal probe thermometer, to limit bacterial growth. Observed some bulk containers of seasoning in kitchen not labeled. Food not in the original packages for use in establishment must be identified with the common name of the food. Observed used wiping cloths sitting on countertops. Wiping cloths shall be used as intended and in compliance with Regulations pertaining to Retail Food Establishment. Hand washing sink leaking from nozzle. Hand-washing sink needs to be repaired so that it does not leak.
MOTOR WAY GROCERY, U.S. 65 SOUTH. Date of inspection Aug.
- Sliced tomatoes (48 degrees F) and sliced onions (60 degrees F) in the prep area in the kitchen are out of safe temperature range. Time/Temperature Control for Safety (TCS) Foods shall be kept cold at a temperature of 41°F or below, as measured by an internal probe thermometer, to limit bacterial growth. Observed pests in kitchen area. Maintain the premises free of insects, rodents, and other pests. The presence of insects, rodents, and other pests shall be controlled to eliminate their presence on the PREMISES by (A) Routinely inspecting incoming shipments of FOOD and supplies; (B) Routinely inspecting the PREMISES for evidence of pests; (C) Using methods, if pests are found, such as trapping devices or other means of pest control and (D) Eliminating harborage conditions. Observed an opening to the outside in dry storage room on the wall. All outer openings of a food establishment should be protected against the entry of insects and rodents. No test strips noted in establishment. Test strips must be available and used to test the strength of chemical sanitizing solutions. Observed unclean wiping cloths and shelf paper lining drawers holding utensils. Drawers should be clean to sight and touch. Hot water not noted in establishment restroom. Hot water needed throughout establishment. Toilet room door is not self-closing. A toilet room located on the PREMISES shall be completely enclosed and provided with a tight-fitting and self-closing door. Observed trash cans containing food residue in kitchen uncovered when not in continuous use. Trash cans containing food residue should be covered when not in continuous use. Permit is not current.
