Inspections are made unannounced by environmental health specialists.

mGURSIMAR-LLC, 2401 E. Sixth Ave. Date of inspection Nov. 4. At least one EMPLOYEE that has supervisory and management responsibility and the authority to direct and control FOOD preparation and service shall be a certified FOOD protection manager who has shown proficiency of required information through passing a test that is part of an ACCREDITED PROGRAM. No soap provided at the hand washing sink in the restroom or the kitchen area. Provide an adequate supply of hand cleanser (liquid, powder or bar) at each handwashing sink. No paper towels provided at the hand washing sink in the restroom or the kitchen area. Provide a supply of individual disposable towels or a continuous towel system that supplies the user with a clean towel or a heated-air drying device at each handwashing sink in food preparation, toilet and warewashing areas. Chicken strips (107 degrees F) and chicken (126 degrees F) in the hot hold unit are out of safe temperature range. Time/Temperature Control for Safety (TCS) Foods shall be kept hot at a temperature of 135.0°F or above, as measured by an internal probe thermometer, to limit bacterial growth. Sausage and cheese (49 degrees F) in the small prep cooler in the kitchen area and sausage/egg/cheese biscuit (46 degrees F) in the small refrigerator by the soda machine are out of safe temperature range. Time/Temperature Control for Safety (TCS) Foods shall be kept cold at a temperature of 41.0°F or below, as measured by an internal probe thermometer, to limit bacterial growth. Observed sticky fly trap tape being used by the prep area in the kitchen. Insect control devices are designed to retain the insect within the device, not be located over a food prep area and in a manner that prevents dead insects and insect fragments from getting into food, equipment, utensils, and linens and unwrapped single-service and single-use articles. Observed used wiping cloths sitting on counter tops in the drink station area. Wiping cloths shall be used as intended and in compliance with Regulations pertaining to Retail Food establishment. Observed trash cans containing food residue being stored uncovered when not in continuous use. Trash cans containing food residue should be covered when not in continuous use. Floors throughout the establishment, especially under the shelving and cooking equipment, are visibly unclean. Clean the physical facilities as often as necessary to keep them clean.

Observation: Observed cold hold not holding temperature 49 degrees. Time/Temperature Control for Safety (TCS) Foods shall be kept cold at a temperature of 41.0°F or below, as measured by an internal probe thermometer, to limit bacterial growth. Regional EH Manager, Keith Blount, visited this facility and confirmed temperatures in the malfunctioning cold hold unit. He spoke with the owner and recommended the repair person take a look again. He requested the owner be present for the re-inspection.

Trash can shall have a lid when not in continuous use and containing food residue. Observation: Floors in walk in cooler need to be cleaned. Corrective Action: Clean the physical facilities as often as necessary to keep them clean.
