PIZZA HUT, 2711 S. Hazel St., Suite B. Date of inspection July 23. Observed bottles of chemicals being stored above food product on shelves. POISONOUS OR TOXIC MATERIALS shall be stored so they cannot contaminate FOOD, EQUIPMENT, UTENSILS, LINENS, and SINGLE-SERVICE and SINGLE-USE ARTICLES by separating the POISONOUS OR TOXIC MATERIALS by spacing or partitioning. Bottles of chemicals were moved to chemical storage area during the inspection. Observed trash cans containing food residue being stored uncovered when not in continuous use. Trash cans containing food residue should be kept covered when not in continuous use.
EXXON ONE STOP, 2901 W. 28th Ave. Date of inspection July 24. Observed containers of food that were poured from a bulk container not labeled with the common name of the food it contains. Food not in the original packages for use in Establishment must be identified with the common name of the food. Observed single service foam cups and single use straws being stored outside of their protective packaging at self service drink station. Single service cups should be stored in original, protective packaging or by other means to protect them from contamination until being used. Walls by the back dry storage shelf are visibly soiled. Clean the physical facilities as often as necessary to keep them clean.

At least one EMPLOYEE that has supervisory and management responsibility and the authority to direct and control FOOD preparation and service shall be a certified FOOD protection manager who has shown proficiency of required information through passing a test that is part of an ACCREDITED PROGRAM. Observed sugar not labeled .Food not in the original packages for use in Establishment must be identified with the common name of the food. Corrected on site. Observation: Observed floors in establishment not cleaned. Corrective Action: Clean the physical facilities as often as necessary to keep them clean.
DAYS INN, 406 N. Blake St. Date of inspection July 24. At least one EMPLOYEE that has supervisory and management responsibility and the authority to direct and control FOOD preparation and service shall be a certified FOOD protection manager who has shown proficiency of required information through passing a test that is part of an ACCREDITED PROGRAM. Provide an adequate supply of hand cleanser (liquid, powder or bar) at each handwashing sink. Hand cleaner was placed at hand washing sink during time of inspection.
